Your username is your first initial and your last name, all lowercase, with no spaces.
If you are the main member, your default password is your 5-digit member number (if you only have 4 digits, add a 0 to the beginning)
If you are the spouse, your default password is your 5-digit member number followed by capital A, with no spaces.
